Your First Visit
It's appointment day! You've already called and scheduled your initial visit - so now what?
Upon arriving at our office you will be greeted by our staff and welcomed to our clinic. Of course there will be some paperwork to fill out. And, if there is any insurance you would like us to check on, this is the time to give us that information.
While you are taking care of your paperwork, you will have the opportunity to see other patients come in for their care. We serve infants, seniors, families, and individuals, and any or all may be present for care sometime during your visit.
Once you have finished your paperwork, the staff will complete your case file and give it to Dr. Dan. He will come and greet you personally and escort you to the consultation/examination room where you will discuss your concerns and health care goals. If together you and Dr. Dan decide that this is the right place for you, he will discuss the examination process, if any x-rays will be needed, and the costs up front.

Therefore we believe in giving you as much information as possible to help you make YOUR DECISION. Then you can expect us to stand behind you and support your choice, and keep you aware of health options that can help you achieve your health care goals.
Following your examination, you will be scheduled for a report of findings where you will return to see Dr. Dan. He will go over the results from your examination as well as to review any x-rays that may have been taken. Because your health, or lack of health, doesn't affect just you, we will often suggest that you bring a friend, or significant other with you for your report of findings.
After you have scheduled for your report of findings and taken care of any copays, you will be given a slip that outlines your first visit. It will include the procedures you received and their cost, any payments, your diagnosis codes, and confirmation of your next appointment.

Report of Findings
Welcome back. During your visit today Dr. Dan will discuss his findings
from your first visit. He will review your initial complaints and
specifically address your health concerns and the options available to
you. If x-rays were taken on your first visit, he will show these to
you and discuss any significant findings.
You, and any support person you bring, will be given the opportunity to ask questions about your case. Dr. Dan will discuss what you can expect from him and the staff as you receive care in our office, along with his expectations from you. Any financial issues will be handled at this time as well.

If at any time during your time in our office you have questions regarding your care, progress, finances, etc., please let Dr. Dan, or the staff know. If needed, additional time can be scheduled make sure we have time to fully address your concerns.
